Heidi Klum is set to change her last name to match that of husband Seal.
Klum has filed legal papers at L.A. County Superior Court asking to take the surname of Seal, whose full name is Seal Henry Olusegun Olumide Adelo Samuel. The decision as to whether she will be allowed to be renamed Heidi Samuel will be made by Nov. 20, 2009.

The pair married in 2005, have two children together, and another on the way. Klum also has a daughter from a previous relationship.
